It might mean that he lacks a heart control. It also could be a general slam try. It will be a disaster if I bid and he meant it as #1 but it is not a disaster if I pass but he meant it as #2. So I pass.

With the 2H bid doubled, N is asking if S has heart control. If not, Pass.

Had there been no Dbl of the transfer bid, I would not know what 5S is but guessing general slam try; go with AK oriented, Pass with soft hand.
