Should be penalty but unless opps are funny it looks unlikely. If they play 4-card majors I suppose p could theoretically have AKQJ.

It's penalties. It's inconceivable that partner has spades and diamonds, couldn't bid on the previous round, and now wants to play at the three or four level knowing of a strong hand on his left.

In the given auction that's particularly clear, but I think that advancer's double should be penalties in all sequences where he passes the overcall and then doubles after opener protects, even this one:
  1   2   pass pass
  dbl  pass 2   dble

... that would still not be conclusive proof, before someone wants to explain that to me as well as if I was a 5 year-old. - gwnn

Yeh, I agree, has to be blood. Partner could have something like KQT and dummy's unlikely to have many entries.

X is penalty, ... at least strongly suggesting going
for blood.

Partner denied a club fit as well, if he has one, he
should have shown the fit the round before, hence
4C is out.

Please note: I am interested in boring, bog standard, 2/1.

Clearly penalty of course.

But this is also a brutal lead problem, no?

Not only is it a brutal lead problem, but it is also a brutal thread title and sub-heading.

I don't think it is a brutal lead problem.

The double is penalties. Pass and lead a trump.
Kind regards,

I think being able to lead trumps from a doubleton on this auction is pretty brutal - for the opponents.
